“Xylitol is a sugar alcohol that is used as a sugar substitute in sugar-free candies and foods,” Dr. Mitos Cabral, DAILY TRIBUNE columnist, explains. “A recent study found that people with higher xylitol levels in their blood had more heart attacks and strokes, but this study did not measure how much xylitol they actually consumed. Our bodies also naturally produce xylitol, so the study cannot prove that eating xylitol caused these events.”
A major 2026 observational study presented at the European Society of Cardiology congress has cast a stark shadow over the popular substitute.
Analyzing data from over 17,700 participants, researchers uncovered a troubling correlation: individuals in the top 25 percent for blood xylitol levels were 57 percent more likely to suffer a major cardiovascular event, such as a heart attack or stroke, over a six-year period compared to those in the lowest 25 percent. Crucially, this heightened risk remained consistent regardless of baseline health factors like age, sex, body mass index, or pre-existing conditions like diabetes and hypertension.
This recent data aligns seamlessly with earlier scientific inquiries. A foundational 2024 Cleveland Clinic study observed the potential biological mechanism at play, revealing that high levels of xylitol can increase the reactivity of blood platelets. Essentially, elevated concentrations of the sweetener can make the blood more prone to clotting, paving the way for adverse cardiac events.
“Occasional use if unlikely to be a concern, but regularly consuming large amounts may be best avoided until more research is available. Ingesting too much xylitol can also cause bloating, gas, abdominal cramps and diarrhea,” Dr. Cabral warned.
It is important to emphasize that these are observational studies, meaning they highlight a strong correlation rather than definitively proving that consuming xylitol directly causes heart attacks. However, as the medical community calls for more rigorous, long-term safety profiling, the current data serves as a compelling warning. When it comes to highly processed dietary workarounds, what seems like a risk-free sweet treat might warrant a much closer look.
