The Philippines clinched the Mr. Gay World title three times in a row when the country’s bet and Mr. Gay World 2021 first runner-up Joel Rey Carcasona was appointed Mr. Gay World 2021 after the resignation of the original winner, Louw Breytenbach of South Africa.

Breytenbach, who was crowned on 31 October 2021, has resigned as Mr. Gay World 2021 over contract disputes. On 29 December 2021, Mr. Gay World Organization made the official announcement of Carcasona’s assumption of the title.

A 28-year-old nurse from Cebu, Carcasona competed in the 2021 Mr. Gay World pageant, which was held virtually and hosted by South Africa. He won the Social Media Presence award and eventually the first runner-up position.

“I would firstly like to express my sincerest thanks to Louw Breytenbach for his work in his time as Mr Gay World,” Carcasona said upon the announcement. “I look forward to using my new profile to push for my advocacy which is universal health care and welfare of the people especially the LGBTQ+ community since it’s very timely, especially now that we are still in the clutches of the pandemic.”

“Helping people provide the right information and encouraging my fellow health care workers to wear rainbow pins, badges, or lanyards so we could have a friendlier, happier and, most of all, a healthier community,” he added.

Carcasona reigns concurrently with Mr. Gay World 2020 Leonard Kodie Macayan, who on the title on 17 October 2021.

John Raspado was the first Filipino to win the title in 2017. John Jeffrey Carlos bagged the title in 2019.
